Point 1: Luck always matters -
Point 2: Quick publishing is just about communication - Efficiency?
Point 3: Trickier. A step change in results has nothing to do with efficiency (P2); could be associated with Luck but better to be a product of effectiveness. Yup, luck still has a role to play but it's not sustainable; needs careful planning and effective execution as you already know!
Let's take a typical scenario, aint that tricky.
Two parts:-
Firstly-the poules
Next - DE's
Without succeeding at the Poules the DE's are a long shot. Forget the 15'eeners; get the 5'evers wrong and everything else collapses unless Lady Luck intervenes!
British scenario - few top seeds meet each other in a poule at an individual Open - result VN-D0 - Only test is at the DE stage.
On the world stage UK fencers have to perform at the poule level but with next to zilch actual experience. Result - First DE if achieved - failure.
Sort the poule mentality first - then start worrying 'bout continuation.
